Welcome! The Arkansas Legal Services Partnership provides these forms as a service to low-income Arkansans and to assist overburdened Arkansas courts. These interactive forms are available for free to low-income Arkansans representing themselves in a civil court matter. Select the form you would like from the list below.

To use these interactive legal resources, select the type of form you are looking for from the list below:

• Child Support Termination
  An easy to use interactive form that will produce a petition and order stating that the child has reached the age of majority and that the support order is terminated by operation of law.
   
  
• Collection Agency Stop Contact Letter
  This easy to use interactive form will generate a letter demanding that a Collection Agency no longer contact you.

• Debt - Late Penalty Removal Request
  This packet creates a sample letter to a creditor requesting the removal of a late payment penalty.

• Debt Validation Request to Collection Agency
  This packet can be used to dispute a debt with a collection agency (not the original creditor) and request validation of the debt.

• Debt Validation Request to Original Creditor
  This packet can be used to dispute a debt with the original creditor (not a collection agency) that has already been placed on your credit report.

• Divorce
  An easy to use interactive form that will produce all the documents needed for a divorce lawsuit. You cannot use this form if you have children or expensive property.

• Expungement (Sealing Criminal Records)
  An easy to use interactive form that will produce all the documents needed to petition for expungement. You cannot use this form if you have been convicted with a felony and sent to the DOC or convicted of a sex crime.

• Guardianship for Minor
  An easy to use interactive form that will produce all the documents needed to petition for Guardianship of Minor (in uncontested cases).  

• In Forma Pauperis (Waiver of Court and Service Fees)
  An interactive form that will produce the three (3) documents needed to request a waiver of filing fees from the court.

• Income Tax Online Filing
  An easy to use interactive form that will produce all the documents needed to file your income taxes for 2010 and file online for free. The Earned Income Credit may result in a refund for many low-income users. You must meet certain income eligibility requirements to use this form.

• Living Will
  An easy to use interactive form that will produce a living will document.

• Order of Protection
  This easy to use interactive form will generate the court documents needed to file for an Order of Protection.

• Power of Attorney for Minor | Paquete de Carta Poder Legal para Menor de Edad
  An easy to use interactive form that will produce a power of attorney for minor document that can be used for for a caregiver of a minor child that is not the custodial parent or legal guardian. This document may be particularly helpful when establishing residency for the child's school enrollment or to provide medical consent. 

• Revocation of Power of Attorney for Minor
  An easy to use interactive form that will produce a revocation of a power of attorney for minor document.

• Security Deposit Demand Letter
  An easy to use interactive form that will produce a letter addressed to your previous landlord demanding the return of your security deposit.

• Small Claims
  This easy to use interactive form will generate the forms and instructions need for filing a small claims action.
